What it Is: The Genesee Valley Health Partnership is committed to finding better ways to improve health and medical care for Livingston County Residents. We are offering "How's Your Health," which can help improve your ability to maintain a healthy lifestyle. Here is how it works:

- The survey takes about 10 minutes. Disponible en español - Either click on th stethoscope or type in www.HowsYourHealth.com.

- Click on the SURVEY option and click on region-Upstate NY/Rochester

- You and your family members can complete the survey by selecting the appropriate age and sex. Based on your answers, you will be given special information personalized to your needs.

- Upon completion, you will also receive an "Action Form" and other forms which will summarize your results. We encourage to share this with your doctor or nurse. Links to county resources are available on the GVHP website.
